The Six Kingdoms seems to have originated in Chaosium's H.P. Lovecraft's Dreamlands RPG sourcebook.
History and Scope
The "Six Kingdoms" is that portion of the Dreamlands which encompasses the Kingdoms of the Skai, Ilek-Vad, Ooth-Nargai & Serranian, Oukranos, the Khalifate of Cuppar-Nombo, and the Principalities of Kled, loosely united by fairly stable political relations, and well-established trade routes by sea or land.

- This organization is referred to numerous times in the RPG sourcebook H.P. Lovecraft's Dreamlands, but not really explained, except that the rest of the Dreamlands largely consist of wandering nomads, and lands ruled by monsters and tyrants.
